Young Alumna Award Nomination Form

Nominators may include a one-page letter of introduction providing a summary of how long they have known the nominee and why they believe they are a worthy nominee, a copy of the nominee’s curriculum vitae (if available), a maximum of two additional letters of support (one-page each), as well as any materials that would provide additional insight into the nominee’s accomplishments (photos, press articles or published information).

Deadline for submissions is Thursday, March 26, 2020 at 5 pm. Complete submissions must be received no later than the deadline. Late or incomplete entries will not be considered.
